1) Application – Declaration<\/a> via gov.gr with text:

“Please accept the request to change my polling station according to my permanent address.”

and the fields must be filled in:
Last Name, First Name, Father’s Name, Mother’s Name, Resident, Street, Date of Birth.<\/i>

In case of incomplete completion, the application will not be accepted. A separate application is required for each adult member. <\/strong><\/p><\/div>

2) Any recent, last quarter document proving the new address of the resident (e.g. utility bills, tax statement, etc.).<\/p><\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t
